If you prefer that the college refers to you by a name that is different than your legal name (on class rosters, your CCC email address, and more), that can be entered here.
If you have a previous name that is different than your legal or preferred name, that can be entered here.
Clackamas Community College does not require a Social Security Number (SSN) for admission, and does not use the SSN for identification purposes for students in general. Although providing your SSN is voluntary, please note that you cannot receive federal financial aid or tax credits without providing a SSN (other forms of Financial Aid are available for students without a SSN).

CCC is required to collect this information below in order to comply with federal reporting requirements and to ensure we appropriately serve the needs of our community. CCC does not discriminate based on any of the following information and does not use it to make admissions decisions.

CCC’s Veterans Education and Training (VET) Center uses this information to connect Veterans and their families with services.
Legal sex designation refers to the legal sex reflected on your government-issued ID and/or the Social Security department. If you are an employee or federal financial aid recipient, your legal sex designation at CCC must match your designation with the Social Security department.

Gender identity refers to your social/cultural/personal identity, which may differ from your legal sex designation.

Hispanic/Latina/e/o/x – a person of Cuban, Mexican, Puerto Rican, South or Central American, or other Spanish culture or origin, regardless of race.
American Indian/Alaska Native – a person having origins in any of the original peoples of North, Central, or South America who maintains cultural identification through tribal affiliation or community attachments
Asian – a person having origins in any of the original peoples of the Far East, Southeast Asia, or the Indian Subcontinent
Black or African American – a person having origins in any of the black racial groups of Africa
Hawaiian/Pacific Islander – a person having origins in any of the original peoples of Guam, Hawaii, Samoa, or other Pacific Islands
White – a person having origins in any of the original peoples of Europe, the Middle East, or North Africa
